Please submit the following information via specific weekly assignment by October 26, 2022 detailing
information on your proposed Leadership project. I need to have evidence of the following information to be
able to adequately support you in meeting the course objectives and giving final approval for your topic/project.
You may use this form and complete as many items as possible. You will, however, need to attach a word
document with more detailed information for #2, #3, and #8 (if applicable). You must complete this form and
provide attachments as necessary. I will send back this form with my comments or suggestions, as well as,
final authorization for you to proceed with your leadership project. When submitting the assignment, you must
submit a word document (not a PDF) since I need to be able to write on your form to provide constructive
feedback.

or the problem significant to nursing? How does this problem impact the work area/department/nursing

by the problem/issue and how to you see that impact (per stakeholder) changing if you successfully
resolve the clinical issue or concern? This is the introduction or significance section of your proposal
4. Provide the specific goal and measurable outcome to successful evaluate your project. This is
critical to the success of the change process, final write up, and evaluation of success. You must give a
number or statistic that you want to achieve to evaluate success in resolving the
issue/problem/concern.

6. Identify the specific change theory you will use to implement the change you want to occur. You will
need to reference the book to determine the specific change theory (Chapter 8, p 186-193).
7. Provide a brief summary of how you plan on doing your project. Provide the steps according to your
selected change theory that you will take to achieve your goal. Be specific and detailed in what you will
do per step. Provide an additional word document if you cannot fit all the information in this space. This
is part of your implementation section of the proposal.

Indicate if you will need to do a survey, questionnaire, or involve something that implies the need for
IRB approval. Please attach a copy of the survey, interview form, or questionnaire if you have one
in mind. You will also need to provide the completed Guidelines for Nursing Project form located in
the IRB section of TU website. Completing this form will identify if there is a need for an IRB review
(expedited or formal).

Measurable Outcomes
Leadership and healthcare in prison setting can focus on quality and performance based on the
following quality indicators and performance measures as identified by Bellass et al. (2022):
Routine Health Assessments
Routine health assessments can be measured by focusing on:
•
the percentage of prisoners receiving physical health examinations at specified time, say 1
month after an admission.
•
the percentage of prisoners seeking dental/ eye examinations within specified time, say 2
weeks after an admission.
•
the percentage of prisoners receiving mental health evaluations within a specified time, say
3 days after admission.
Access to Care
Access to care can be measured by focusing on:

the percentage of prisoners with urgent sick calls and who access primary care within a
specified time, say within the first two days of falling sick.
•
the percentage of prisoners referred for urgent sociality care within a specified time, say
within three days to 1 week.
•
the percentage of prisoners with light sick calls seeking medical attention within specified
time, say hours (eg 24 hours) or days (eg 7 days).
Infectious Diseases
Infectious diseases can be measured by focusing on:
•
the receipt of influenza, pneumonia, and hepatitis B vaccination on prisoners thought of
being at high risk because of their age or other chronic conditions or both.
•
Pre- and post- HCV RNA tests for prisoners previously diagnosed with hepatitis C and
undergoing anti-retroviral treatment.
•
Percentage of prisoners with HIV/ AIDS with specific viral load counts.
•
Percentage of prisoners with HIV/ AIDS under the prescribed potent ARVs.
•
Percentage of prisoners with HIV/ AIDS under the prescribed PCP prophylaxis within
specified duration, say 5 months of low CD4 cell count.
•
Percentage of prisoners with HIV/ AIDS that had CD4 cells count performed for a specified
number of times in the previous time, say 1 year.
•
Percentage of newly admitted prisoners holding documentations of tuberculosis screening.
•
Percentage of prisoners with positive tuberculosis tests and have successfully completed
curative therapy within a specified time, say 6 months.

days after laboratory tests.
Mental Care
General
•
Treatment plan for all the prisoners with mental health needs.

treatment phase.
•
Bipolar disorder: the percentage of prisoners with bipolar 1 disorder and specified lithium
levels.
•
Schizophrenia: the percentage of prisoners receiving antipsychotic medication.
•
Suicide: the percentage of prisoners who attempt suicide with a score of MH higher than
say 5.
•
ADHD: the percentage of prisoners receiving treatments based on stimulant medication.
Physical Health Conditions Care
•
Cardiometabolic; the percentage of prisoners with chest pains and have an ECG.
•
Respiratory: percentage of prisoners with persistent asthma.
•
Musculoskeletal: the number of prisoners who have undergone back pain assessment
within 2 months.
