Patient History
Ryan is a 23-year old, athletic male. Over the past week, he noted increasing fatigue, sore throat,
earaches, headaches, and episodic fever and chills. These symptoms have prevented Ryan from
being able to run his customary 25 miles per week. He decides to see his doctor, Dr. Kalamazoo,
at the end of the week.
Upon examining Ryan, Dr. Kalamazoo notices that his throat and tonsils are erythematous, and
his cervical lymph nodes are swollen. Dr. Kalamazoo immediately orders a complete blood
count (CBC) and differential. Remember that the differential is an addition to the CBC that
quantifies each TYPE of white blood cell and expresses it as a percentage.

Questions:
1. Which values in the CBC and differential are abnormal, if any? (2 points)
2. How would you describe the erythrocytes with regard to color and size? Use appropriate
terms. (2 points)
3. Is there any platelet satellitism? Why? (1 point)
4. Identify the three large, light blue cells on the peripheral blood smear. What are they, and
which cellular characteristics led you to your answer? (3 points)
During physical examination, Ryan mentions to Dr. Kalamazoo that his roommate has been ill
with infectious mononucleosis for the past two weeks. Dr. Kalamazoo asks him if they ever
drink from the same glass or bottle, and Ryan replies that they sometimes do.
5. Why would Dr. Kalamazoo ask this question? Is there a relationship between infectious
mononucleosis and Ryan’s clinical signs and symptoms? (2 points)
